About the Role 

Satori Health is seeking an experienced Licensed Practical Nurse (LPN) to join our team as a Health Plan Coordinator. In this role you will act as a patient navigator, communicating essential health updates and collaborating closely with patients’ healthcare teams. Your expertise will play a key role in guiding patients toward achieving their long-term health goals, ensuring they have a comprehensive understanding of their care plans and available resources. 

 

The successful candidate must have a valid LPN license with a minimum of 5 years of diverse clinical experience. A strong commitment to patient education and proactive healthcare is essential, along with comfort in utilizing various technology platforms to support virtual patient care. This position allows for a flexible work environment, with opportunities to work from home as well as in-office days at our Satori Sunpark Office based a rotational schedule and business needs.

Responsibilities 

Provide informative and engaging patient education to ensure patients feel comfortable, supported and well informed about their healthcare 
Guide patients through their healthcare support systems, assisting them in setting goals for optimal health outcomes 
Conduct comprehensive reviews of patient care and history in EMR and Netcare, with a strong focus on practitioner notes and relevant documentation, including lab work, diagnostic reports, consults, and past medical histories 
Maintain regular communication and collaboration with healthcare practitioners to enhance patient support and optimize care 
Contribute to team goals, consistently striving for excellence in patient outcomes and overall service delivery 

 

Requirements 

Licensed Practical Nurse in good standing with CLPNA 
Minimum of 5 years of diverse experience in various nursing disciplines, including critical care, urgent care, pediatrics, women’s health, mental health, and/or chronic pain 
Experience working with chronical illnesses, critical care, internal medicine, and/or specialty practices 
Minimum of 3 years of experience working in a clinic and/or family practice setting considered an asset 
Sound knowledge of medical terminology, pathophysiology and reviewing lab and diagnostic results 
Previous experience with virtual healthcare consultations is considered an asset 
Strong English language skills required for clear communication in both written and verbal interactions related to job responsibilities 
Advanced knowledge of Netcare and an EMR systems, including Accuro, Med Access, Health Quest, and/or Telus CHR 
Expert in MS Office Suite, with a willingness and ability to learn new IT software applications

Committed and enthusiastic about patient education and providing an exceptional patient experience 
Ability to work effectively both independently and collaboratively within a team 
Adept at communicating complex information clearly and precisely, both orally and in writing 
Demonstrated ability to effectively manage and resolve conflicts 
Flexible and adaptable to shifting priorities and requirements 
Strong analytical skills with the ability to think critically and independently 
Capable of working in a fast-paced environment with a high degree of accuracy and attention to detail
